Dr. John Spencer Ellis: Divorced Men Get Two Pieces of Advice, and Both Miss the Point

Get a good attorney. Talk to someone. Both are sound. A Las Vegas longevity expert says the advice men never receive is the one supported by mortality data across seven million people.

LAS VEGAS, NV — When a man's marriage ends, the advice arrives quickly and it is always the same two items. Get a good attorney. Talk to someone. Both are reasonable. Neither addresses what the research says is happening to him.

"Nobody says get your bloodwork done," said Dr. John Spencer Ellis, an internationally recognized authority on men's health, longevity and human performance. "Nobody says fix your wake time before you do anything else. Nobody says count your drinks honestly for two weeks. And those three are the ones with mortality data behind them."

The evidence for a third phone call

Two independent meta-analyses reached identical findings. Sbarra and colleagues (2011), pooling 32 studies covering more than six million people, found a hazard ratio of 1.30 for mortality among divorced adults, with stronger effect sizes in men. Shor and colleagues (2012), analyzing 104 studies, arrived at HR 1.30 as well.

A systematic review of 7,881,040 individuals found divorced and separated men carried higher all-cause mortality, plus stronger cancer and cardiovascular mortality risk, than divorced and separated women. The Japan Collaborative Cohort Study found the same elevation in men and no equivalent trend in women.

"Two separate research teams, using different methods on different datasets, landed on the same number," said Dr. Ellis. "That is not a soft finding. And the male skew appears consistently enough that it deserves to change what we tell men."

What is happening while he handles the legal side

Dr. Ellis describes a sequence he sees with unusual consistency.

Sleep degrades first, and it matters more than men realize — the majority of daily testosterone release occurs during sleep. Six fragmented hours becomes the new normal within two months, and by month five he has forgotten the baseline moved.

Cortisol does not resolve, because divorce is a stressor without an end point, running eighteen to twenty-four months across proceedings, financial separation and custody.

Fat redistributes to the abdomen — and that tissue is not inert. It secretes inflammatory cytokines including interleukin-6 and tumor necrosis factor alpha, and it contains aromatase, the enzyme that converts testosterone into estrogen. The weight gain does not sit alongside the flatness. It creates it.

Alcohol rises quietly. Increased consumption is repeatedly identified in the literature as a male-typical externalizing response to relationship breakdown, escalating precisely when self-monitoring is weakest. It suppresses muscle protein synthesis, fragments sleep architecture, raises blood pressure and lowers testosterone directly.

And the markers drift in silence. Blood pressure, lipids, fasting glucose and hs-CRP all deteriorate without producing a single symptom.

"He is dealing with attorneys and custody schedules, which is where his attention should be," said Dr. Ellis. "Meanwhile something entirely mechanical is happening in the background, and nobody has told him it is happening."

Why the second piece of advice often does not land

The instruction to "talk to someone" is good and men largely ignore it — but Dr. Ellis notes there is a structural reason beyond stubbornness.

"Male distress after divorce rarely presents as sadness. It presents as irritability, compulsive overwork, risk-taking, emotional numbness and withdrawal. So when a man is told to talk to someone, he genuinely does not recognize himself in the description he has been given. He is not sad. He is furious, exhausted and working eighty hours. The men in the most difficulty frequently look like the ones handling it best."

He adds one signal men consistently misfile. Erectile changes are common during this period, and erectile dysfunction typically precedes a cardiac event by two to five years — the penile arteries measure 1 to 2 mm against 3 to 4 mm for the coronaries, so identical endothelial disease narrows the smaller vessels first.

"That is not embarrassing. That is a cardiovascular warning issued early. It belongs with a physician, not a website that ships a prescription and asks nothing else."

There is a social component too. Male friendship in midlife is largely activity-based and proximity-dependent, and divorce disrupts most of it. Holt-Lunstad's meta-analysis of 148 studies covering 308,849 participants found strong social relationships associated with a 50 percent increased likelihood of survival.

The advice he gives instead

Dr. Ellis is explicit that his recommendations sit alongside the legal and mental health guidance, not in place of it.

Book the full panel — blood pressure, lipids, fasting glucose and HbA1c, hs-CRP, thyroid, morning testosterone and vitamin D. Ideally while employer coverage is still in place.

Fix the wake time. Seven days a week, seven to nine hours in bed, morning daylight within an hour of rising. It sits upstream of cortisol, testosterone and every decision made the following day.

Count the alcohol, then halve it. Not eliminate — halve. Two honest weeks of counting first.

Then train. A 2024 network meta-analysis in The BMJ covering 218 studies and 14,170 participants found walking or jogging produced SMD 0.62 for depression against active controls, with the authors concluding exercise could be considered alongside psychotherapy and medication as a core treatment.

"The order is the intervention," said Dr. Ellis. "Men who attempt all four at once abandon all four inside a fortnight and conclude they lack discipline. They do not. They attempted a sequence nobody sustains from a starting point of exhaustion."

Two coaching pathways

The Health, Longevity and Aesthetic Optimization Program for Men 40+ delivers exactly this sequence: a 90-day personalized engagement with comprehensive intake covering sleep, stress load, alcohol and medications; bloodwork guidance alongside the client's own physician; a protocol across training, nutrition, sleep architecture and stress physiology sequenced so sleep and alcohol stabilize before training load increases; and twelve weekly one-on-one coaching sessions.

The Escape the Rat Race Coaching Program addresses a different obstacle. Divorce frequently arrives with financial pressure, and the reflexive male response is to work harder — which eliminates the exact hours every step above requires. That program converts decades of existing expertise into a simpler, more profitable, location-flexible income model that returns time and control rather than consuming them.

"Many men run them in sequence," said Dr. Ellis. "Rebuild the physiology first, so the bigger decisions get made by a man who is sleeping properly and thinking clearly. Then restructure the work that keeps undoing the progress."

Dr. Ellis is a coach and educator, not a physician or licensed therapist, and does not diagnose, treat or prescribe. His coaching is designed to work alongside medical and mental health care. Men experiencing persistent low mood or thoughts of self-harm are directed to licensed professionals; in the United States, the 988 Suicide & Crisis Lifeline is available free and confidentially, 24 hours a day.
